軍人恩給法ニ據リ恩給請求ノ件　福岡県宗像郡津屋崎村　平民　元海軍一等水兵勲八等小瀬萬太郎　元治元年十月生　免除恩給年額金五十八円也　右明治二十九年一月十五日服役満期ニ付兵役ヲ免シタルヲ以テ恩給請求差出候ニ付審査ヲ遂クル処軍人恩給法第五条第一ニ拠リ恩給ヲ受クヘキ資格アル者ト確認ス依テ証拠書類相添進達候也　明治二十九年五月十四日　海軍大臣　内閣総理大臣　団第四八号　恩給請求書　萬太郎儀　明治十五年十二月二十八日二等若水兵被申付尓来十三個年服役同二十九年一月十五日服役満期ニ依リ免役被申付候ニ就テハ軍人恩給法ニ拠リ相当ノ恩給下賜度証拠書類相添此段　元佐世保鎮守府海兵団　佐世保鎮守府司令長官　柴山矢八殿
