第二六号　朝第一七九八号　遊就館額面献納ノ件　靖国神社　明治二十八年四月一日　指令案　伺ノ通リ　靖甲第一二号　額面献納ノ義ニ付伺　故陸軍歩兵大尉松崎直臣油絵肖像ノ額一面右ハ日本橋区通二丁目九番地松雲堂主松井嘉起ヨリ別紙ノ通願出候ニ付御許可相成遊就館ニ掲ケ綬覧ニ供シ度此段相伺候也　明治二十八年三月二十四日　靖国神社宮司　加茂水穂　陸軍省副官　山内大佐殿　献納願　一額　一面　故陸軍歩兵大尉松崎直臣氏肖像　五姓田芳柳草　勝安房伯貨入　右献納御渡御許容ノ上ハ遊就館内ヘ御陳列致度別紙雛形相添ヘ此段奉納上候也　右　日本橋区通二丁目九番地　松雲堂主　願人　松井嘉起　明治二十八年二月二十日　靖国神社宮司　加茂水穂殿
