呉鎮第四〇九二号 上申 本職儀 舞鶴水雷団検閲之為メ住返滞在共一週日間ノ予定ヲ以テ出張致度候条電報ヲ以テ御認許相成度此段上申候也 明治三十四年七月十五日 呉鎮守府司令長官柴山矢八 海軍大臣山本権兵衛殿 海総第二八八五号 帰京御届 明治三十四年七月十八日進達 小官儀 去月十九日出発佐吉保鎮守府及竹敷要港部ヘ出張中之処昨十七日帰京仕候召此段及御届候也 海軍技師工学博士石黒五十二 海軍大臣山本権兵衛殿 呉＠＠五五六号ノ三 御届 本職儀 予テ御允許済横須賀海兵団并ニ海軍省ヘ出張中之処本日帰団致候条此段御届仕候也 明治三十四年七月十九日 呉海兵団長滝川具和 呉鎮守府司令長官柴山矢八殿 横医第二〇八号 帰著月日 用務官氏名 七月二十日 兵学校生徒志願者体格検査ノ為メ出張中 軍医中監石川詢
